Sending Documents

At certain times, I will need supporting evidence regarding your medical status

Please DO NOT ask your GP or specialist to send reports or letters to me directly. Instead, please ask them to send them to you and then you keep hold of them until I need to see them.

In general, collect paperwork during the year and bring it all in paper form to your medical.

If I need to have anything sent to me between medicals, I will always ask you.

Additionally, if I do need to see a certain document, please ALWAYS SEND IT AS A PDF file as this is the only file type I can accept. All other file types such as JPEG photos will be rejected I’m afraid.
